Gynaecomastia, sometimes referred to as “man boobs,” is an enlargement of breast tissue in men. It is estimated to affect a significant proportion of the male population at some point in life, and is commonly diagnosed in teenage boys and older males. Signs include prominent breasts or lumps of fatty tissue below the nipples, which may sometimes feel tender or sore.
This excess tissue can be surgically removed.
At Skinnotion, gynaecomastia surgery is performed by Dr Peter Tran, MED0001668228, General Registration.
Individual results vary and are not guaranteed. Outcomes depend on factors including tissue type, healing, and post-operative care.
What Causes Gynaecomastia?
True gynaecomastia involves enlargement of fatty tissue, whereas pseudo-gynaecomastia involves enlargement of both fatty and glandular tissue. Several factors may be associated with this:
- Hormonal Imbalance
While testosterone and oestrogen are present in both males and females, testosterone is typically higher in males and oestrogen higher in females. When males experience an increase in oestrogen levels, it can cause breast tissue to swell. Hormonal changes during puberty and adulthood, including decreasing testosterone levels associated with ageing, may contribute to gynaecomastia.
- Weight
Higher oestrogen levels are linked to weight gain, and as breast tissue is largely composed of fatty material, weight gain can also enlarge it.
- Age
Gynaecomastia occurring during puberty usually resolves by the time boys turn 16 or 17. In older males, higher levels of body fat may be associated with increased oestrogen production and gynaecomastia.
- Less common causes
In rarer cases, excess breast tissue growth in men can be associated with conditions or injuries affecting the testicles, given their role in testosterone production. Other potential but less common causes include anabolic steroid use, thyroid conditions, lung, pituitary or adrenal gland tumours, liver or kidney disease, Klinefelter Syndrome, or alcohol or drug use.

Is Male Breast Reduction
Surgery Right For Me?
Some men who experience gynaecomastia report it affects their confidence or comfort. Suitability for surgery is assessed individually during consultation, considering your health history, the nature of the tissue involved, and your goals.
What To Expect From Gynaecomastia Surgery :
The procedure involves making incisions, typically around the edge of the areola or within the creases of the chest, to remove excess fat tissue (true gynaecomastia) and/or glandular tissue (pseudo-gynaecomastia), as well as excess skin where required. Incision location and length (typically 1-2 cm) depend on the extent of tissue removal required. Incisions are closed with dressings or bandages, and techniques aim to minimise the visibility of resulting scars.
Male breast reduction surgery can be performed under local anaesthesia with analgesia in clinic, or in hospital if preferred. The procedure typically takes around 1.5-2 hours.
Your doctor will provide detailed post-operative instructions, which typically include wearing a compression garment or elastic bandage to reduce swelling and bruising, taking prescribed oral or topical medications, and monitoring for symptoms that require medical attention. Recovery time for this procedure is generally 4-6 weeks. Complete rest is usually advised for 2-3 days after surgery, with strenuous activity avoided for 1-2 weeks. You may experience pain, tenderness or tightness in the treated area initially. Your doctor will remove dressings at your first follow-up visit.
Removed fat cells do not regenerate, so results are generally long-lasting, though remaining fat cells may still enlarge with future weight gain.
You will be required to undergo a medical evaluation and any necessary tests prior to surgery, along with a physical and psychological assessment to help determine suitability and reduce risk. You may be asked to adjust current medications, and may need to discontinue anti-inflammatory drugs and herbal supplements. Smoking or vaping should be stopped completely at least 2-4 weeks before surgery, as this can impair tissue oxygenation and increase the risk of tissue damage. You will need to shave your chest and underarms prior to surgery, and your doctor may advise fasting for 6 hours beforehand. Arrange for a family member or friend to accompany you to the clinic or hospital, drive you home, and stay with you for the first 24 hours.
What Are The Risks?
As with any surgical procedure, gynaecomastia surgery carries risks, including bruising and haematoma, infection, breast asymmetry, fluid accumulation in the breast, temporary or permanent changes to muscles, blood vessels or nerves, and temporary or permanent changes in nipple sensitivity.
This list is not exhaustive. All risks will be discussed in full during your consultation.
